What Does Black Speck In The Urethral Opening Of A Child Indicate?

My son is 5. He has been complaining he cant pee. When looking at the penis, it looks like there is a little black speck in the opening. The orifice has went from being an open whole to what looks like a small slit. After pulling it apart he was able to pee again. It looks like it is closing off. What could be wrong with him? Is there need for emergent concern? How can it be fixed?

Pediatrician 's  Response
Hi, I think there is issue with the foreskin covering the opening.. Yes if it is troubling the kid definitely it has to be looked into . If what I am thinking based on your explanation, then it could be a requiring a minor surgery... that is circumcision. It's just a one day hospitalization
