hi, i was diagnosed l5s1 by doctor based on MRI in 2009 after taking doctors advise i have started physiotherapy i was perfectly well for last 3 years. again low back pain has been aggrevated and I got my MRI done, now diagnosed L4-5s1 and the impression is sequencing disc spine segment cyery behind l4 on lt side, compressing lateral recess xl4 nerve on lt side with disc at l5s1 tent with degenrstion changes l5s1 with osteoplytosis l2l5 with miedclic bulge l3-4, l4-5 please advise me for the suitable treatment

Hi,
Thanks for writing in.

As you are aware, you have degenerative disc problems at more than one level. There are neural foramina on either sides of the bones of the lower back. The nerve roots pass through these foramina and when there is degenerative disc disease, there is pressure on the exiting nerve roots and this may cause severe pain.

Initial management is by physiotherapy which you have already been doing for 3 years. If there is severe pain due to additional disc degeneration then you may need to take neurogenic pain relievers. Surgery is recommended only for those with severe pain which does not respond to medications taken for months. Laser assisted surgery is also available which is minimally invasive.
