What Causes Whooshing Noise In Ear?

About 4 years ago I started hearing a swooshing noise in my right ear when I would push in the bathroom. I have never followed up on it. For the last 4 months I have had an uncomfortable pressure in about a 6 inch diameter around my right ear with worsening headaches.I have been seen for possible ear infections and was told my ears were not infected although one nurse said my eardrum looked kind of bulging. Now my swoosh in the bathroom is louder and I feel disoriented and dizzy frequently and am having trouble consentrating.my headache also seems to be spreading to the back of my head as well. Any help as to what this could be?
