What Causes Syncope After Undergoing A Surgery For Pancreas And Gall Bladder Removal?

My wife has had major surgery for cancer. her Pancreas, Gall Bladder and spleen were removed. she has been recovering a month now and has started to black out on short trips to the bathroom . ten or 15 steps and she just glazes over for a few minutes. What might be the cause? should I be worried about clots?
