Hallow Dear,

You have faced what is called as Membranous dysmenorrhoea.

Usually, during menses, the lining of the uterus (endometrium) is expelled out of the uterine cavity in small bits. Therefore the bleeding continues for about 4-5 days.

In membranous dysmenorrhoea, the endometrium is expelled out en mass; hence it retains the shape of the uterine cavity. The large chunk that was expelled during your menses was having the shape of the uterine cavity; hence it was Y shaped and appearing like tongue. Since such a large piece has to be pushed through the cervical canal, the uterus has to contact very forcefully which causes severe spasmodic pain.

Usually, such incidence is very occasional and needs some painkiller. If you are having it often; you may take progesterone dominant birth control pills which will thin out the endometrium. Reduced volume of endometrium may give you relief.

Please report to your Gynaecologist.
