I occasionally suffer from a severe itchonly located on my arms .There is no rash associated with it but I do scratch until the skin is broken. It seems like the more I scratch the worse it gets .I have tried all of the over the counter itch creams which have no effect on this.I have been to a dermatologist and she couldnt determine what this could be.Scabies was ruled out

As you have mentioned in your history that you have severe itching and scabies is ruled out than most probably you are suffering from fungal infection. For this you have to take some antibiotic preferably cap doxycycline 100 mg twice daily after meals for at least 5 days along with tab levocetrizine or tab fexofenadine twice daily after meals for itching with tab rantac or tab famotid twice daily before meals. In addition to this you have to use clobit-gm or valnerate-c cream for local application. Avoid scratching of local part, apply vaselline or coconut oil as it will be helpful to reduce the severity of itching.
