What Causes Radiating Pain In The Upper Quadrant Of Abdomen?

hello, Since Dec 26 I have been experiencing intense radiating pain in the left upper quadrant of my abdomen. Cuts my breath, can't move, shooting pain over the 10 on a pain scale, I'm noscious and no appetite for 4 days now, I'm winded, and I feel bloated. I've been to ER and they did blood work, urine tests which cam back fine, a D Dimer test that came back positive. They gave me a shot of Toredol and an injection of blood thinner and sent me home as the CT tech was gone for the night. I returned next day for the CT scan but I'm allergic to IVP dye so they performed a nuclear respiration xray test instead. A different doctor then informed me that the nuclear xray test showed no PE. I asked about the positive D Dimer and he said it was unreliable and that he himself test positive and he's fine. They sent me home with a prescription for Toredol and said I had a probable pulled muscle. Problem is...the pain is still there and returning to insane pain again, but now it's shooting across my abdomen, cutting me in half. I feel it radiating up in my back, inside the left shoulder. This AM I started feeling small shooting pains on the right side. When the pain flares on the left, I get a shot on the far side near the back on my right side. I am becoming more and more uncomfortable. I am returning to ER today but want to be better informed as to what tests should be done...Please help

* Primary requirement is high resolution ultrasound of the abdomen &
pelvis & hospitalisation in the hospital .
