What Causes Pot Belly With Weak Body In Toddlers?

Hi there, Greetings. Out son, 18 months now, is weak overall but has a pot belly. His tounge is very white around the center as well. He has normal appetite though. Is pot belly normal for toddlers with otherwise weak body? Anything else we should be concerned about?
