Firstly u have to see either one tonsil has enlarged or both.or one tonsil has enlarged more than other.if there is enlargment of both tonsil may be unequal suggest acute tonsillitis if one one tonsil enlarged it gives suspicion of lymphoma. Start with tab amoxicillin Clav ,antihistamine, tab pan, betadine gargle and tonsillectomy and biopsy if unilateral enlargement
