General Surgeon 's  Response
Hi.
Thanks for your query and an elucidate history.

Your present complaint of slight pain in the left testicle which is lower than the right may be due to the following reasons::
Large varicocele,
Mild epididymo-orchitis.

The left testicle is usually at the lower than the right one by anatomical predisposition.
So do not worry about its level.
Lower back pain 2 years ago could have been due to any problems , not related to the present problem.

I would advise in such a patient the following:
Physical examination for clinical diagnosis, to be done either by a General Surgeon or an Urologist. This gives faster diagnosis with the help of ultrasonography and color doppler examination of the scrotum.

Further treatment will depend upon the actual findings
Till then use a scrotal support.
Avoid strenuous exercises or work.
I find this answer helpful
