Hypothyroidism is the major cause for intense shoulder pain as gradual fibrosis of the shoulder joint that causes a limited range of motion. Although common over the counter drugs (ibuprofen or acetaminophen) relieves such pain , adequate thyroid hormone replacement is the primary treatment and often dramatically reduces pain. So you should go for the replacement of thyroxine level with a specific synthetic thyroid hormone (such as levothyroxine).
