What Causes Heavy Vaginal Bleeding?

I am 33 yrs old. 5'1 and last weight was 127. I have a regular period every month. It arrives between day 28-31. This past month I had a UTI and went to the doctor right away. I was prescribed Macrobid.
My period was 10 days late with repeated negative test. I had slight cramping on and off since a couple of days before expected period. I starting spotting, light pink that Saturday night and when I woke up the next morning, it was still only light pink and only when I wiped. I went about my day expecting it to get worst, but when I changed my tampon, there was barely anything on it. I called my dr's office the following Monday to tell her what happened and she told me I most likely hadn't ovulated. A couple of hours after I talked to her, I started spotting again. It was a little heavier, with more of a red color, but still only when I wiped with on and off light cramping. Wednesday, I called my dr's office to let them know what happened since the Monday and they told me to come in for a transvaginal ultrasound. They did the u/s and found a very small functional cyst, about 2.5cm. My endometrium wasn't consistant with a period and my ovaries were not enlarged. They told me that's what has been causing the cramping and spotting and prescribed Provera 10mg. The next day, the bleeding was a little heavier, enough to wear a pantyliner, but still light enough to only have very little blood on it. The cramping became more steady and stronger, but not unbearable. In fact, it felt like my menstrual cramps. Friday night it got heavier, still less then my normal period, but more like a light period. That continued until the Saturday. At about 3 o'clock that afternoon, I went to the restroom and had what looked like a small sac, with this liver looking substance coming out of it with bloody stringy tissue. The sac wasn't red except for the blood that got on it. I never passed any chunks or any other blood clots. Just the one thing. I never started the Provera, because I researched it and heard nothing but horror stories. And most doctors let them pass on their own, since they are fairly common. The doctor seemed to rush through my diagnosis and wanted to go with the first thing he thought of. I have researched everything I could on follicular cyst and its my understanding that a cyst this small with no enlarged ovaries should not be causing the bleeding. Cramps maybe. It's been 8 days and I still have bright red spotting, not as heavy as yesterday before I passed the tissue/sac, and cramping. Is there a possibility the doctor could have misdiagnosed me and it could be something more serious?
