What Causes Heavy Bleeding & Clots During Periods?

Hello dr. I had a d&c on August 28,2014 and the gestational age was determined at 10 weeks and 3 days. I had a previous abortion and suffered from complications namely an infection due to retained tissues( placenta). I had no symptoms until about six weeks post procedure where I was in considerable amount of pain, bleeding heavily and passing clots. This required a follow,up D&c to rid of the whatever is in there. Going back to my recent procedure. I have barely bled, throughout the two weeks. I had a little bit of spotting the day of the procedure. And then it stopped. I had one incident of a gush of blood on the 30th of August. And then nothing until September 12th where I started passing brownish( I m sure it s blood and not discharge) material. I have not passed any clots either. I decided to take a home pregnancy test on the evening of the 15th of September and I couldn t quite tell if it s a pure negative or a faint positive. The vertical line was extremely faint and almost not there. The morning of the 16th I took another test and the faint line was more visible, nonetheless, still,faint. I am concerned as I don t want to go through complications like my previous experience. I cannot go see a doctor where I am now. I was hoping to know when I should be concerned. I am not experiencing any pain,fever, heavy bleeding, cramping or clotting. I am paranoid about having any left over material. How should I proceed from here? Shall I wait for a week before taking another home pregnancy test? Thank you

I have read your notes.
With the D&C procedure some women experience bleeding just for a day or two and this is normal. What is of concern is the brownish discharge that you are having since 12th September. Brownish discharge that you have can be due to several reasons like ovulation, cervical erosion, retained pregnancy tissue etc. It is extremely important that you have an ultrasound to assess for the possibility of retained pregnancy tissue in the uterus as this may cause infection. You are aware of this complication.

You must get an ultrasound done now.
Normally pregnancy test remains positive for 2-3 weeks after abortion. Pregnancy test can be negative even in the presence of remnants in the
uterus. So don't do any pregnancy test. It is not helpful.

Hi,
Thanks for the query. I understand your concern.
Blood hcg testing gives earlier confirmatory report about pregnancy.
About retained products.... usually after sugical mtp, bleeding dies not stop completely.. unless all the products are expelled out. The intermittenr bleeding indicates either post abortal hormonal imbalance or ealy pregnancy with thretened abortion.
