Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

I am sorry for the health problems your 23 years old son is going through. Working in a busy ER is always associated with high adrenalin explaining his heart rate being extremely high especially if he is used to drink caffeinated drinks at the same time.
Next, nose bleeding could be due to 2 main reasons:
- Dry weather
- Anemia (due to malnutrition from being too busy)

The good part of being ER team is to be always in close contact with doctors. He can explain his case to the doctors there and possibly run a few diagnostic tests:
- Complete blood count to rule out anemia
- Monitor his blood pressure and heart rate and record the results
- Get a good ENT consultation with specialist to rule out nasal issues
-Advise to keep well hydrated.
-Use room humidifiers to avoid dry air.

Hope I have answered your query.
Let me know if I can assist you further.
