Dermatologist 's  Response
Hello. Thank you for writing to us

I suggest a possibility of schaumburg's purpura/ pigmented purpuric dermatosis.
Schaumburg's disease is common on lower legs , ankles and may go up to the shin level.
Pathogenesis is not exactly understood but it is said to be due to capillaritis with local hemosiderin deposition.
The appearance is like cayenne pepper and the spots do not blanch on pressure.
The spots are usually asymptomatic apart from cosmetic concern.
Treatment options that have been tried include with topical steroids, Oral calcium dobesilate, Oral Vitamin C, Phototherapy etc however none of these produces consistent results in all.
I suggest that you visit a dermatologist in your vicinity for a confirmatory diagnosis as well as appropriate prescription.

Regards
