What Causes Black Spots Around The Nails And Knee Pain?

I am a 75yo female with a 30 year history of RhA, I suffered a MI 6 weeks ago and was hospitalized, during which time I developed an ulcer measuring 5mm x 5mm with a sloughy base that probed to the bone on the 5th digit of my right foot. I also lost 7kg, and have since gained 1kg and am eating well now. I discovered I have black dots around my nails in both my hands and feet, I also experience unremitting pain in my right knee and foot which is worse around the area of the ulcer. I am currently prescribed 5mg Prednisolone taken twice a day, 75mg Methotrexate and Folic Acid taken weekly and also 5mg of Warfarin taken daily

Based on your query, my opinion is as follows.
1. Black dots around nails indicates areas of ischemia.
2. Commonly seen in association with bacterial endocarditis.
3. Will require blood culture, complete hemogram and echocardiography. Being on corticosteroids and immune suppression, symptoms maybe less.
4. Meet your doctor to get the tests done. You will require antibiotics therapy based on blood culture and sensitivity.
