Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

The worm is most likely just the lining of your uterus that was shed when you menstruated. If you still have it, I would take it to your doctor who can send it to the lab for testing. Also, it’s important to check if this "worm" came out of your vagina or your anus. If it came from your anus, you might also be having problems with your bowels, eating, or your stomach.

PCOS is a cause of infertility or difficulty getting pregnant. If you know that you have this diagnosis and you do not have regular periods, this is definitely a possible reason that you have not gotten pregnant. If I was your doctor, I would send the "worm" to the lab for testing, check your hormones to confirm you are releasing and egg every month (ovulation), check that your ovaries are working well, check your pituitary gland and your thyroid.

I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Monica Kapoor
