Hello!

Thank you for the query.

"swollen" or better enlarged stomach can be caused by bloating due to bad diet, constipation, bacterial infection of intestines. Back pain with stomach problems usually indicates gallstones. Some bloating can also appear due to gallstones.
Back pain can be also caused by spine problems or urinary tract infection/stones.

I suggest you to have some tests done. Start with abdominal ultrasound, blood work, liver tests, pancreas tests and urine analysis. If you are aver 50, and bloating seems to be your problem, also colonoscopy should be considered.
Please also consult orthopedist.

Hope this will help.
Regards.
