Urologist 's  Response
Hello and welcome to HCM.
As an Urologist, let me advise you, that removal is advised , only if, the scrotal doppler ultrasound scan, shows very poor, or, no blood flow in the affected testis, usually due to torsion testis, or a severe trauma.
If the other testis is normal, there's no cause for concern, as sperms production from the testis, will not be affected.
Male hormones will also be normally produced. To clear any doubt, you can get a semen analysis test, done prior to surgery. You're welcome to send a copy of scan reports here in my name, for an expert opinion.
