Taken Antibiotics For UTI. Urine Report Shows Pus Cells, Occult Blood. Concerned About Recurrent Infection

Hello, My mother was diagnosed with urinary tract infection for which was on oral antibiotics for more than 3 months. The infection would decrease but not clear off completely. Last month, she was hospitalized as the infection spread to kidneys resulting in pyelonephritis, sepsis and hypoxia. She was in a very critical condition but fortunately she recovered after spending 15 days in ICU and 15 days in general ward. While in hospital she was given carbapenem class of antibiotics on IV. When she was discharged she was put on oral antibiotics. Within 20 days after discharge, her infection increased, creatinine level rose and she was admitted in a hospital again. A stent was placed in one of her ureters and emphysema in kidney was treated. She was discharged after 4 days. She is on oral antibiotics. My concern is her urine report, she still has 30-40 hpf pus cells ,presence of occult blood, although creatinine is somewhat near normal. My concern is the rise of infection again causing life threatening situation and she is a diabetic since last 10 yrs. So blood sugar management is also part of the treatment. My questions are 1) how to treat her to clear the infection completely ( which antibiotics to administer)? 2) Will Intravenous administration work better and faster than oral ? 3) What tests should we get done regularly (apart from urine and blood) so that increase in infection is detected early on? 4) Is it ok to get a CT scan done with the stent in the ureter? 1) how to treat her to clear the infection completely ( which antibiotics to administer)?
we cant tell which antibiotic now. The specific antibiotic requires culture of sample and its sensitivity to different antibacterial agents. Till then broad spectrum antibiotics are used as emperical therapy.

2) Will Intravenous administration work better and faster than oral ?
Yes. Because of 100% bio availability and directly reach the desired level. No affected by 1st pass metabolism.

3) What tests should we get done regularly (apart from urine and blood) so that increase in infection is detected early on?
Urine and blood tests should be done on regular basis as they can detect the infection. Get done CBC, ESR, CRP, FBS, PPBS, RFT, electrolytes and urine routine on regular basis. These will help in monitoring her health including infections.

4) Is it ok to get a CT scan done with the stent in the ureter?
Yes you can use. No problem.
