Swollen, Tender Glands Under Jaw. Pain And Tenderness Above Adam's Apple. Any Help?

Hi, I have had swolen, tender glands on my right hand side, under my jaw. They have been on and off for a month. A couple of days ago the pain and tenderness moved to just above my adams apple. Ive been feeling around the area and just noticed a small lump there. I can only feel it when I pinch the skin above the adams apple. Its small and moves in my fingers a little.
Any help would be much appreciated
