Hi, Pain in testicles may be due to pathology in testis itself and the pain may be referred from other organs to the testicles. Infection in the large intestines or colon may also give rise to dragging pain in the testis. It is therefore necessary to rule out: 1. Any swelling, infection, cyst in the testis by performing an ultrasound examination of scrotum. 2. Treat the colitis which is associated with episodes of diarrhoea and if the pain is relieved, it may be due to the colitis. 3. See if he has any urinary issue, in such cases pain in the testicles may be present. 4. Since, there is family history of malignancy of colon, it would be better to perform colonoscopy for any polyp, familial polyposis coli etc. Such conditions in the initial stages are harmless but in later life may be life threatening. Hope for a better diagnosis and management.
