Hi,

In your case, the symptoms seem due to presence of Liver cirrhosis. Presence of fluid in the abdomen, fatigue, cough and breathlessness due to pressure of abdominal fluid on the lungs may definitely due to liver cirrhosis. Normal blood work and even liver enzymes some times may not be contributory many a times. X-ray chest without presence or very minimal presence of pleural fluid may also be normal. Similarly, a small amount of fluid in the abdomen may be easily missed in abdominal X-ray or even CT SCAN of torso.

But, I am not able to understand, how, it was diagnosed that you are having abdominal fluid. Therefore, in this case, to reach at a proper diagnosis, you may need further examination and investigations, like aspiration of the abdominal fluid for biochemical, histopathological studies etc. You may also need, further studies of heart condition by echocardiography, further studies of kidney functions, thyroid profile & liver isotope studies or even liver biopsy etc.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
