Suggest Treatment For Left Ovary Pain And Pelvic Pain

Hi, I ve been experiencing left ovary pain followed by pelvic heaviness and a dull lower back ache. Using tampons is painful as well as intercourse. Also, every now and then I ll get a pain underneath my left rib cage that radiates to my back. I have been extremely tired no matter how much sleep I get. I have been pretty constipated, going once every few days or so. I went to the gynecologist last week and they did a transvaginal ultrasound and he saw a solid mass which he thought to be stool blocking the view of my left ovary that also had blood flow surrounding the solid mass. He also saw a couple of small fluid filled cysts on my right ovary. He then thought I just had constipation pains and sent me home to drink 10 oz of magnesium citrate followed by 3 large glasses of water. He said if the pain hadn t subsided within 24 hours to call back. Not to my surprise the pain was still there so he scheduled me for a ct scan and another transvaginal ultrasound for yesterday. I got to radiology and drank the contrast, they did a urine pregnancy test which came back positive so they couldn t follow through with the ct scan. I then got sent back to the gynecologist to take an additional pregnancy urine test and blood test. I m scheduled to go back again tomorrow for another set of urine and blood tests to see if my hcg is rising and if so it means that I am indeed pregnant. I just can t understand how I m pregnant if I had the mirena iud inserted on March 5th and he did not see that I was pregnant during the transvaginal ultrasound. I ve been pregnant twice before so I know what it feels like, and this feels nothing like that. I ve had an ovarian cyst removal surgery 2 years ago off of both ovaries and had a daughter 2 & 1/2 months ago. What could this be that my doctor is missing, please help. I m so tired of being in pain.

Hello dear , thanks for writing in. I would have liked to have more clarity on when you had your last delivery and when mirena was inserted. And if you are breastfeeding your child.
However, as I do not have the scan reports and your symptoms and the description of scan findings and blood test do tell me that you could be pregnant . But this pregnancy is not in your womb but it has happened either in your fallopian tube ( commonest ) or your ovary ( less common ). Many a times ectopic pregnancy ruptures if left undiagnosed and it can be fatal. I must say you be got your tests just in time.
I m not quite sure what those fluid filled cysts are in the right ovary. But I do assume that is of not much importance as it could be pregnancy related corpus luteum cyst which will go away.
Mirena does prevent intrauterine pregnancy but it doesn't have protective effect against pregnancy in the fallopian tubes.
I m more than sure that what you have is ectopic pregnancy and the treatment depends on the size of the mass, if baby heart beat is visible, beta hcg levels and few other things. And it can be surgery or by medical method using injections depending on above mentioned factors.All I can say is, although it is unfortunate that you had ectopic pregnancy, it is easily treated and you will be back to normal after treatment.
Do remember to report to ER if you feel giddy or if pain increases anytime and not to ignore it.
