General & Family Physician 's  Response
Hello dear welcome to HealthcareMagic
Regarding your experience with 2 blood clot for the past 3yrs with a diagnosis of splenomegaly and saddle pulmonary embolism with associated low alkaline phosphatase and aspartate transaminase I am suggesting that you have possibly developed a liver cirrhosis which has led to development of the above complications

In view of this please do a an abdominal ultrasound and abdominal CT scan to rule out this liver disease

Meanwhile please see a cardiothoracic surgeon and hepatologist specialist for expert clinical assessment Councelling and possible surgical interventional procedures

Moreover you will benefit from the following treatments

Embolectomy
Invasive endoscopy

I hope this helps Regards
