Suggest Treatment For Hemorrhoid, bloating And Brown Discharge From Rectum

Hi I am a 31yo female with a history of female issues. I have had two children via c-section. Sept 2011 I was diagnosed with a 5mm endometrioma on my left ovary. At the same time I was also diagnosed with my first ever hemorrhoid. I was told it was small and no big deal. It itched like crazy and at times bled. I was scheduled to have my ovary removed in Nov. I asked for one more US prior to and the endometrioma was gone. Just disappeared. I then had a Mirena IUD inserted Dec 14 to help with my endometreosis and irregular periods. Ever since my hemorrhiod seems to have worsen. Along with constant discomfort, cramps and bleeding from my IUD. I also have severe bloating. Sometimes i feel like I am laying on a ball. I have now started to pass a slimy brown discharge after a bowl movement which i think is coming from my rectum. I don't know what to do and feel like a complete mess. It seems as if my doc gyno who I have seen for 13yrs doesn't believe me or feels as its nothing. Please help. Thanks Liz

General Surgeon 's  Response
Hi.
Thanks for your query.
Noted the history and understood your concerns.
Multiple problems of different specialities like Ovarian endometrioma that vanished, pile to start with but gradually slimy brown discharge from rectum, spasms due to MIrena and so on indicate a proper clinical evaluation by a Gastroenterologist or a Gastro surgeon by the following ways:
First of all get a proper per-rectal digital examination, ano-proctoscopy so that the grade of pile and any rectal polyp or sulcer or proctitis can be known, to be followed by a colonoscopy and biopsy to know the primary cause so that a proper management can be done.
Tests of stool and blood do help a lot in proper diagnosis.
If there is any infection or so, get a course of an antibiotic and metronidazole along with probiotics.
Sitz bath in a tub with warm water ives soothing of the anal region.
Laxatives helps in smooth passage of soft stools.
Avoid the foods and beverages that increase the problems.
Gynecologist;s opinion will help further for the Gyne issues.
CT scan of the whole abdomen does help.
Further management will be as per the reports and clinical evaluation.
