Suggest Treatment For Alopecia Areata In Scalp.

HELLO, THIS IS ASHOKE BOSE FROM KOLKATA, INDIA. I HAVE BEEN SUFFERING ALOPECIA AREATA IN MY SCALP FOR LAST ONE YEAR. I HAVE HAD INJECTION ON MY SCALP, USED COVERIT 2% LOTION AND FOLLIDERM TABLETS BUT IMPROVEMENT HAS BEEN SHOWN A LITTLE. PLZ SUGGEST ME .

I appreciate your concern regarding the bald patch on scalp and i will try to answer your queries in best possible way.

Alopeica areata is one of the common condition which causes patchy hair loss.
Though your definitive diagnosis rests upon direct physical examination by a dermatologist, i would insist you to share your pictures so i can help you in a better way.

Alopecia Areata is an autoimmune condition where your own immune system attacks your hair follicle leading to hair loss. Usually the condition is harmless and self resolving but mandates treatment because of cosmetic concern.

Alopecia areata is known to be associated with history of chronic allergic diseases like asthama, atopic dermatitis and other conditions like thyroid disorders, hypertension etc. Thus, in non-responding cases or extensive cases i do suggest investigations.

In my patients of alopeica areata, i decide the treatment protocol depending on the history, associated conditions and area of involvement and as the results are appreciable after few months of treatment, thus patience is of utmost importance.

I would advise intralesional steroid injection therapy every 3 weekly as the first line treatment for you.

Additionally, i would prescribe oral corticosteroids and biotin rich multivitamin tablets.

For local application, i suggest application of Minoxidil 5% solution and a steroid (clobetasol) mixed with salicyclic acid solution.

In resistant cases, i also apply concentrated phenol over the patch every 3 weekly interval.
