General & Family Physician 's  Response
low prolactin may be due to various causes mc is pitutary tumour in males others may include TB Sarcoidosis(presents with pulmonary complaints),histoplasmosis(fungal infection)head injury, hemochromatosis(heme accumulation in body due to excess which may be due to increased destruction or increased formation ,lymphocytic hypophysitis(autoimmune pitutary inflammation) but since his value is just below 2 ng/ml also his testosterone low normal range iwant to ask his insulin level was fasting or postprandial? if its fasting then he may be having some thing like acromegay or cushings if features are there THESE ARE ALL possible causes BUT I THINK
you know why the psychiatrist has asked you prolactin- because antipsychotic medication increase the prolactin levels and psychosis cause decrease in prolactin by incresed action on dopamine 2 receptor
SO IN ALL YOUR SON'S BIPOLAR HAS DECREASED HIS PROLACTIN AND IT WILL COME NORMAL WHEN STARTED ON ANIPSYCHOTICS
