General Surgeon 's  Response
Hi,

Feeling of tightness of throat or neck, worsening on sitting down or leaning back and lying down, 4 mm cyst on ultrasound, indicates one of the following reasons:
* The commonest being inflammation of the muscles, tendons or at the hyoid bone or thyroid cartilage.
* Other reasons can be muscle spasm due to stress, anxiety, wrong posture at table or while at work or in home while working or watching TV or so.
* Cyst of a size of 4 mm can not cause these symptoms hence surgery will not help to alleviate your symptoms.
* I would advise you the following in such a situation:
Consult an ENT Surgeon for proper clinical examination, investigations like Endoscopy of pharynx, an actual palpation examination of neck and particularly of the hyoid bone and thyroid cartilage. The treatment will be as per the cause found. Till then you can take an anti-inflammatory medicines.
Avoid stress and anxiety.
Identify for any faulty posture and rectify.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. T. Chandrakant, General Surgeon
