Thank you for the query.
As per the history, you are 53 yrs old (gender not mentioned), hypertensive (controlled with Telmisartan 40 mg) and diabetic (on no medications).
There are two methods of controlling your blood sugar levels - Lifestyle modifications and medications. Lifestyle modifications include Dietary modifications (which comprises of avoidance of refined carbohydrates, saturated fat and carbonated beverages) and daily exercise regimen. If you are a smoker, you must quit it, as soon as possible (since that aggravates the risk of cardiovascular disease).
Now, coming to medications. It is very difficult to advise a medicine over the internet. A variety of anti-diabetic medicines are available - Oral drugs and Insulin. To prescribe a drug, I must know about your complete blood sugar profile (Fasting blood sugar, 2 hrs post prandial blood sugar and HbAic). HbAic is a very important index and it gives an idea of the control of blood sugar. If it is below 7%, medications are not required usually. If it is more than 8.5-9%, some physicians prefer giving insulin. A number of oral drug are available and as per patient's physical profile and tolerability they are prescribed. Metformin, is the commonest first line drug to be given (however, it cannot be given in deranged renal profile). Other groups of drugs are - Sulfonylureas (Glimeperide, Gliclazide), DPPIV inhibitors (Vildagliptin, Linagliptin), Alpha glucosidase inhibitor (acarbose, voglibose) and PPAR gamma group (pioglitazone). The list is not exhaustive.
It is best to visit your primary physician, who can perform a physical examination, and prescribe you drugs accordingly. Continue taking Telmisartan/Losartan, as they are beneficial in a diabetic patient.
Hope this helps you out.
