Hi

Your brother in law is most probably suffering from stage 4 lung cancer which has spread to his brain.
For pneumonia he needs to be treated with sensitive intravenous antibiotic.
Treatment at this stge is only basic supportive care and if it has spread in his brain then MRI brain followed by radiation to brain may help.

Regards

DR De
