Schizophrenia. Unnecessarily Aggressive, Negative Thoughts, Wrong Interpretation Of Facts. What Is Advisable To Do?

Dear sir,my wife is 43 years old. some 14-15 years ago she was diagnosed schizophrenia but we were not knowing what this illness is all about .As was advised by the doctor that time medicines were taken for for a week or so .I do not remember the names of medicines , Due to lack of knowledge we did not continue with the treatment and continue making adjustments ,cooperating and bearing her strange behavior..Me and my son now22 have been suffering a for the last 20 years. finally with a Psychiatrist I discussed her strange behavior like thinking that whole world is her enemy,always suspicious, always propagating negative thoughts , unnecessarily aggressive at times ,wrong interpretation of facts etc. She has been diagnosed with FZo.OZF42.2,the psychtrist has adivsed Olimelt 5mg,Asprito 15 mg and Fludac 20 mg. She has been taking these medicines for the last 20 days and there is a magical change in her behavior and our life has become livable but now she is having joint pains,loss of appetite and shortening of breath. suffered a lot due to this reason the medicines stands discontinued for the last 5 days and there is significant improvement in her health condition in regards to the side effects ,the doctor has advised not to discontinue the prescribed medicines and take pain killers etc.for joint pains.Me and my son loves her too much and want her get out of this illness as soon as it is possible so that all three of us can enjoy our life.Please advise ,whether she will be alright /fully recovered .As the side effects are now reduced with the discontinuance of medicines but we are about to restart the medicines tonight but if the symptoms of side effects re-occur can we discontinue with these psychotropic medicines for few days again.Your valuable advices on the subject are solicited. I totally understand your problems, however, it seems to me that the problem has not been treated for many years and I am glad that you have planned to initiate treatment tonight. I believe that there is a lot to gain from persisting on the medications, as the side effects if any, are likely to not remain for more than a few days to a week. So kindly follow your psychiatrist's advice and adhere to medications till you visit with your psychiatrist the next time around. It will help with stabilizing the mood and you will see a drastic change in the way your family responds to the issues you have been facing.

Hi Rajinder.
From what you have written, i could gather that your wife has had a long period of what we term as 'untreated psychosis' (appx. 14-15 years). Now most studies done in this regard agree that a long duration of untreated psychosis is signifies a poor outcome subsequently. COnsidering that, it is really heartening to know that your wife had responded to Olanzapine 5, Aripiprazole 15 mg and Fluoxetine 20.
The former two are atypical antipsychotics which address the core symptoms of schizophrenia (your wife's diagnosis is ?? F20.02 with F42.2, which is "paranoid schizophrenia with OCD mixed thoughts and acts"). The latter might cause loss of appetite, very rarely joint pain and shortness of breath etc.
In view of the wonderful response your wife has had to medications, it is advisable on my part to discuss all the relevant issues with your psychiatrist and arrive at the most practical and helpful drug regime for her.

It seems she has a good chance of functional recovery, and I wish her a good health.
