With the history of pain while swallowing or talking, the most probable diagnosis goes towards a bacterial infection of the tonsils, which is common in this season. If I were her doctor, I would go with a throat swab culture/sensitivity test. and while waiting for the report, I would prescribe a broad spectrum antibiotic for her along with an anti inflammatory, a decongestant,an anti allergic and a local antiseptic gargles for her. The antibiotic might have to be changed depending on the culture and sensitivity report of throat swab. With this regimen for five to seven days, she should be completely relieved of her symptoms. However if the symptoms persist, and continue to increase in intensity ( which is less likely), she has to be reviewed by an ENT specialist for further management.
