Assuming you underwent key hole surgery for a paraumbilical hernia then there are many possibilities.
1. The process of fibrosis which causes irritation in that region.
2. Infection in view of diabetic state.
3.If you happen to be obese then the chance of the mesh fixators getting loose causing dislodgement of the mesh and recurrence.
