Infant. Runny Stools, Red And Painful Bottom. Serious?

Hi. Good evening , my 8 months old boy, he had runny poo for 5 day now, before. I just use some sudocrem each time for nappy change, but yesterday I found he had a red and painful bottom, today seems not getting better, he must feel pain, and today he poo for 6 time, each time just a small runny water, what can I do now.do I need to take him to 24hours Gp ?

you should give him extra fluid and ORS ,
monitor his hydration status
if there is any blood or fever or child starts vomit every thing then immediately visit you doctor
nappy rash can be treated with zinc containing ointments
change his diaper after every loose stool
how nuch urine he is passing
