For sugar control , insulin should be tailored according to the weight of the patient. Right now i cant comment about it. sometimes we need to add another oral agent to control the blood sugar levels but again it is strictly according to patient characteristics. second thing is ,we need to look whether it is fasting or random blood sugar which is not controlled. we need to look other systemic symptoms too.
