How Can Severe Pain In The Collar Bone And Shoulder Be Treated?

Dislocated shoulder in a horse wreck years ago. Gives me trouble periodically. Started steroid shots and they helped but Dr will no longer give them. xray showed arthritis and some other long named thing. The severe pain is in the tendon thing around collar bone meets shoulder. Recently I have noticed a lot more popping and noises from shoulder. What options for relieving pain are out there?
