Gaining Weight, Elevated Cortisol Level, Has Abdominal Discomfort, Feels Lost. What To Do ?

My daughter has always been active and healthy and slender. She will be 21 years old this fall. Over the last three years she has put on a lot of weight. She won't tell me how much but I suspect roughly 50 pounds plus. She has a poor appetite and many days only eats one meal a day. She is adopted so I have no family medical history. She looks classic Cushing's Disease and her cortisol level was elevated but the swab tests and taking steroid night before and blood drawn in AM were negative. She tells me she is continuing to gain weight. She was evaluated with bloodwork for gluten sensitivity but it was all negative. She states she feels like she is in a fog sometimes. She is often nauseous with no appetite and was vomiting on average three times weekly without any preceding factors. A few weeks ago, I started her on Prilosec daily and a probiotic. Since that time, she has only vomited once but continues to complain of abdominal discomfort. I haven't seen it but she reports that she has had blood in her stool. She is also emotionally labile compared to three years ago. I don't know what to do next. A concerned, loving parent.

Pediatrician 's  Response
Hi,

Thank you for your query.

I am a bit confused by the information provided by you. At one place you mention that her cortisol level was elevated and follow it by saying that "the swab tests and taking steroid night before and blood drawn in AM were negative". If the cortisol level is raised and she is complaining of blood in stool and abdominal discomfort then she requires a detailed clinical examination and an ultrasonography for further guidance.

I hope I have answered your query to your satisfaction.

Wishing your daughter all the best.
