Fever,pain Behind Ear, No Rash. Taken Ibrufen, Tylenol And Azithromycin. Diabetic. Remedy?

My husband has been to his doctor twice this week and called once. A fever on and off up to 102, no fever for 24 hours on day 6, so went back to work. Fever started again. ibuprofen and tylenol alternating. Today day 9, at doctor after feeling really bad this am with fever. pain behind right ear when fever is up. has not thrown up, but sometimes nauseated. No rash, neck doesn't hurt, fever breaks and in a big sweat, clammy when has fever, his age is 60. Works full-time second shift as a material handler. I work full time first shift and we share care giver for our adult son, paralyzed in an accident 7 years ago. My husband doesn't get enough sleep. He normally is in good health and a good sense of humor. Sometimes I think he gets short tempered and i think he needs meds for that but he doesn't think so. We have been married 40 years. We are concerned about him being contagious. We have 4 small grandchildren we are supposed to see Saturday, probably not a good idea. I wanted to take him to ER, but he doesn't want to stay there, so he said no. Any ideas of his problem. Dr. ordered CBC today, but insurance has it sent to a reference lab, so don't know when results get back. He was given a 3 day antibiotic today called azithromycin 500 mg, 1 a day for 3 days. He is diabetic and takes meds for that.

If I were your doctor for this type of fever , I would come up with these possibilities , these includes.
1 Typhoid fever as fever present from the last 9 days and not so high so may be due to typhoid as also there are cold , excessive sweating . Diagnosis can be confirmed by blood examination for Widal test.
2 Tuberculosis , diagnosis can be confirmed by X ray chest with physical examination by a physician as your husband is already a case of diabetes , so very prone to lung infection.
3 As you mentioned that there is pain behind ear when fever so least possibility is infection of parotid gland. Diagnosis can be confirmed by physical examination by an E N T .
4 Idiopathic fever so get in blood for C B C and E S R .
In my opinion consult a physician and get his help as physical examination is important.
