Hi,
From history it seems that your father might be having fissure in ano as he gets severe pain while defecation.
Infected piles also should be ruled out.
Apply antibiotic cream locally.
Consult surgeon and get examined.
Meanwhile continue with the medicines, prescribed by your doctor.
