Hello. Thanks for asking from HCM.

I can understand your concern. The restlessness could be due to

: Brain infection - The brain infection can cause psychological symptoms along with neurological symptoms. Usual symptoms are either hyperactivity like restlessness, irritability, abusive behaviour or hyperactivity like dullness, unresponsive, decreased social behaviour. It usually occurs with fever, fits, some focal deficit like weakness/loss of speech, headache.

If your brother symptoms resemble above, it could be due to infection. He will get relief after treatment of infection. From your question, much details about infection are not available so I can not comment about treatment. You can provide more details by asking another question or you can ask his doctor about it.

: Drug side effect - Levipil or Levetiracetam can cause psychological symptoms like behavioural problems, suicidal tendencies, sedation or decreased alertness. It can be confirmed by ruling out other cause and any visible brain damage (frontal, temporal and parietal lobe) detected by MRI brain.

The prescribed dose in your brother is 2000 mg per day which is normal dose. Do not give it without prescription and more than 3000 mg daily.

If drug is the cause, taper drug gradually under doctor supervision. Do not stop it your self. It can be replaced by another suitable drug like Carbamazepine, Valproate etc.

Hope it will help you.

Thanks. Take care.
