Dizziness And Lethargy After Switching From Nebivolol To Inditel H, From Glimsave To Dianorm M For HbAlc 9.9. History Of Diabetes. Advice

Hi, I am 60 year old. I am diabetic since the past decade and was on medication. Recently doctor analyzes my HbA1c which was 9.9, and it is unsatisfactory. My doc switched me from nebivolol to Inditel H (telmisartan 40 mg and hydrohlorothiazide 12.5 mg), and from glimesave m1 to dianorm M. Now I feel so lethargic and dzziness.

Hello,
Thanks for the query to H.C.M.Forum.
Still HBA 1C is high as far as concerned for diabetes.
Since drugs have changed , so hope this will work .
Your 's main complaint is dizziness and lethargic feeling , as these two symptoms are due to side effects of hydrochlorothiazide as this is a diuretic and loss of fluid with certain mineral develops dizziness and lethargic .
2nd reason may be due to hypotension ( due to inditel H), so please get it check.
3rd reason may be due to hypogycemia due to dianorm M , so get in blood for sugar level.
Consult your treating physician and get his opinion.
Good luck.

General & Family Physician Dr. Pavan Kumar Gupta's  Response
Hello,
You are suffering from uncontrolled diabetes which can cause lethargy and dizziness.
Your symptoms of lethargy and dizziness can also be due to side effects of telmisartan and HYPONATREMIA caused by HYDROCHLORTHIAZIDE.
Hydrochlortjiazide can also cause increase in cholesterol and blood sugar levels.
Ideally HYDROCHLORTHIAZIDE is avoided in diabetic patients.
You should get your electrolytes chexpcked up and discuss with your doctor for stopping HYDROCHLORTHIAZIDE and continue taking telmisartan or change it entirely.
You also need a revision of your anti diabetic medicines because there is not much difference between glimmisave and dianorm as both belong to sulfonylurea group of drugs. In fact there should be addition of other class of anti diabetic drugs in your current medications like voglibose or the current dose of sulfonylurea be increased or a better drug like GLIPTINS be added.
There is lot which can be done regarding control of your blood sugar levels.
Discuss with your doctor or alternatively you can talk to a diabetologist for the better prescription.
Thanks
