Diagnosed With IBS, Get Gastric Distention, Feeling Of Fullness In Abdomen With Eating, Have Difficulty In Weight Loss, On Thyroxin. Remedy?

dear sir, i am 56 yrs, general physician by profession running a private clinic, i have been diagnosed as having irritative bowel syndrome for last 15 yrs, now for last few days anything i eat produces too much gaseous distention and feeling fullness of abdomen, with passage of gas through mouth. Previously this was with select food items (lentils, cabbage, garlic) . i have tried prokinetics, enzyme preparations, probiotics,initially anything i tried, gave relief for sometime and then become ineffective in due course,i am taking 100 mcg of thyroxin daily for hypothyroidism ,rosuvas10 mg ,and temisartan with amllodipne for BP now BP T3, T4, TSH , LFT, KFT, CBC are all within normal limits. Although I am overweight BMI 33, and try my best (daily walk and exercise bike for 1/2hr each)but unable to reduce the weight.what else should I do to get some relief

Hi,

Nice to know that you are a General Physician and you have given nice detailed history.
Since you are getting temporary relief with the medications your have mentioned like Pro-kinetics, enzyme preparations and probiotics for gases and eructations.
This indicates the following issues as possibility:
Stress and anxiety is one of the major reasons as one gets aerophagia while eating or so and thus the flatulence.
Aerated drinks.
Gas producing products like nuts.
I would advise you the following in such a situation:
Get an Upper GI Endoscopy and Barium studies for esophagus, stomach and small intestine as follow-up.
H Pylori test.
This will give a chance to find out any local cause and get corrected if found.
As for the weigh reduction: You seem to be on a correct path of exercises which you may need to increase gradually.
Diet plan in consultation with a good Dietitian helps a lot.
If the dieting and exercises do not help then bariatric surgery is one good option and you may please consult a Surgeon doing Bariatric Surgery.
Continue Medications s per the requirement for thyroid and other medical conditions.
