, since one of the rare possibilities would be that of autonomic dysfunction (dis-regulation of the automatic nerves that control things like blood pressure, temperature, digestion, etc), and he can order the battery of tests needed in such cases. Again, it is quite rare, but it is in the differential diagnosis.
