Chest And Jaw Tightness Followed By Numbness In Right Forearm

I have been having some chest tightness for about 4-5 hours. About 2 hours ago I have had tightness in my jaw. Shortly there after, I have had periods of numbness in my right forearm. The chest pain is often worse after lying down. I am 35 and exercise regularly. My heart rate has felt somewhat erratic.
