Can Vancomycin Cause Red Man Syndrome And Low BP?

Tomorrow will be one week since I had a right side craniotomy microvascular decompression with nerve section for trigeminal neuralgia and during my surgery i was given vancomycin to fast or the infusion rate was to fast causing me to develop red man syndrome they said my face neck and chest got bright red, then my blood pressure was extremely low and i barely had a pulse. What i want to know who is at fault here for this? And is the hospital liable for this medication error?
