Hi,
If you are always regular with your cycles, have never missed a period and are sexually active without any contraception, this spotting could mean an implantation bleeding. You can test for pregnancy 3-7 days after the missed period with a fresh early morning sample of urine using a home pregnancy test. The pain can be due to very early pregnancy or premenstrual. A trans-vaginal sonogram done a week after the missed period along with a clinical evaluation by a specialist clarify the issue. Hope this helps.
