25 Years. Weight Loss, RBS Showed High Blood Sugar Of 390. Medication?

Heloo i m a mbbs doc n doing pg prepration.my brother is also doing mbbs from china. his age is 25 .last some time he is loosing his weight n he is done a rbs accidently got high sugar next day morning he was done rbs from labs getting 390 blood sugar.......my qus is what tx shoud we r started

Hello and welcome to HCM,

A fasting blood sugar needs to be done before starting any treatment.
Urine needs to be tested for ketone bodies.
A raised blood glucose, weight loss and ketonuria suggest Type 1 dibetes mellitus.
The treatment depends on the blood glucose levels.
If they turn out to be very high insulin needs to be given to immediately decrease blood glucose levels.
Later biguanides are the drugs of choice.

MODY is diagnosed if diabetes is there in 3 generations. so, confirm family history, if it is there, most likely it is MODY. initially it can well be started with life style modifications and oral hypoglycemic drugs like metformin. you have not given other details, like BMI,lipid profile etc.

Hello,
First and foremost is to diagnose the condition.
It could be MODY or LADA ( latent autoimmune diabetes of adults ) .
The main difference between two is that people with LADA have various autoimmune antibodies like islet antibodies or GAD antibodies.In LADA one also has low levels of C- peptide and fasting insulin levels.
People suffering from LADA also find that their sugar levels deteriorate much faster than MODY people.
MODY has a much slower progress and has no GAD or ISLET antibodies.
They also have normal fasting C - peptide and insulin levels,because the flaw is in secretion of of insulin in response to rising sugar levels and their fasting levels are normal.
Moreover their is strong family history present on MODY cases.
You must consult a diabetologist and get the correct diagnosis and treatment.

Dear sir/madam

MODY will present like the symptoms that you have mentioned, but invariably it require insulin over the period of five years.
The weight loss is due to insulin deficiency, so you need to supplement with insulin.
