What Risks Are Involved In Adult-onset Seizure?

Question: My wife delivered a baby girl by c-section on 9th May. She did't have any complication and her bp and sugar were normal. On the day of discharge she suddenly had severe headache and then she had convulsion due to which she had to be admitted to ICU. Multiple tests have been done but no abnormality has been found. Doctors have been agreed that it is a grand mal seizures elderly onset as she didn't have any history of epilepsy. As situation improved she had been discharged from hospital. A MRV has been done as advised and report suggested PRES with no abnormalities of veins. She now has been taking Eptoin 100 (1 in the morning and 2 tabs at night) and Predmet 24 mg for 3 days followed by Predmet 12 mg for 3 days along with regular post partum medicines. Now my questions are:
1. Whether there is any risks that we'll have to be worried? If yes, please suggest the remediation.
2. Whether she can resume the breastfeed while taking eptoin as prescribed?
We'll be really grateful if u please help. Thanks in advance for your support.

First, congratulations for your baby girl.
I am sorry for the situation her mother is in.

If she has been diagnosed with adult-onset seizure, then, :
1. There is no risk you should be worried about. The only thing she might have is another seizure (convulsion) attack. If this happens, try to put your wife at a safe place, where she cannot hurt herself and wait for the attack to go on its own.

After the attack, give her medicines as prescribed and let her rest.

Try to hire or take another person to take care of your wife and the little baby. Should be careful if the attack would start when she is holding the baby.

Otherwise, everything else is ok.

2. Unfortunately, women who are taking eptoin (or any other seizure drugs) are not advised to breastfeed to avoid severe complications to the babies. So, I do not advise her to start breastfeeding. Unless, her doctor allowed it.

Thank you so much for the answer, I'm bit relieved now. As rightly mentioned by you, it has been diagnosed as adult onset seizure. I have some followup questions, though:
1. Will there be possibility of another seizure attack, even though she is under anti convulsion medicine?
2. I've just received the confirmation from our paediatrician that she can breastfeed her baby, and set the timing for the same according to the intake timing of eptoin. Just wanted to know, what will be the side effects that may arise?

Hi back,

1. The aim of the treatment is to minimize chances for getting another seizure attack. Although our hope is not to have any, there is still (low) risk to have another attack, at least for these first days of starting treatment. In the future, hopefully, she will be fully recovered.

2. It is ok if the pediatrician was ok for her breastfeeding, then, she can continue breastfeeding. According to XXXXXXX Academy of Pediatrics, phenytoin is considered "usually compatible with breast-feeding". The main concern is methemoglobinemia. You can discuss this issue further with the pediatrician for proper care.

She will have to take anti-convulsion medicine according to her health status. If she responds well to the treatment and no convulsion attack is noticed, then, her neurologist might lower the dose until take them off.
