Question: Hi, My wife is 31 yrs old mother of a 9 month old daughter. She is having OCD, i have also visited a doctor in the past who has diagnosed her and he has confirmed it. she tend to repetitive clean hand, always scared of germs getting into everything she do or touch, take long time for bathing and scared that mosquito repellent can get into the milk or food she prepare for baby. Baby is still breast feeding so we are not sure if she can take medicines for OCD. Can you please guide us on how to take it forward for the treatment and medicines.Also, if you can provide us the prescription if you think medicines are required. I am based in noida so how can i visit you in case it is possible.

Important aspects of your query are: • 31 year old female • Known case of OCD and Hypothyroidism. • Concern related to treatment of OCD during breastfeeding. OCD is chronic illness and require long term treatment with medicines and psychotherapy (like CBT). But Medicines are avoided during breastfeeding, as they increase risk of development defect 1-3 % by passing into the child through breast milk. But same time if symptoms are very severe then treatment can be started in breastfeeding period also. As poor health of mother affect the child care and bonding of mother-child. In that case medicines with good safety profile during breastfeeding are used like paroxetine. Apart from that non drug treatment like CBT is preferred which do not have risk for child.
